Well, if you never get your reward, try a complaint to Survey Police. That was the only way they ever responded to me and paid me.

I GOT MY $50 Visa card
The code to use in order to access my reward was put into my account sometime between Friday and today and I redeemed my reward. It expires 5/31/2010 but I will surely use it online by then. Waiting a month for the reward is really pretty darned good! So it all worked out at the end!
