Used this link in Firefox and filled in all the info on the page, clicked submit and said it required an email address yet there was no field for that. I called them and they told me to use a different browser, which I did (Chrome) , went to their site and it showed a field for the email address. I was able to successfully send for this sample. Thanks, lea62!
