Date: Wednesday May 10, during business hours.

Location: Otivo Inc., South of Market in San Francisco
Time to complete this survey: less than 5 minutes
Payment if scheduled for web site review: $75 or $100
If selected to participate, you will be asked to sign a consent and confidentiality form, and you will receive the payment in cash at the conclusion of the session. The session will be videotaped.
