Need to locate my cousin's phone #! Help!

Can someone here help me locate my cousin's phone number? I know his name, address, and the company he works for. I even know his contractoe license number, lol, but I need to call him at home. I'd like to avoid calling his workplace, if at all possible. There was a really good look-up tool posted here awhile back. I thought I'd saved it, but now I can't find it. I'd love to have that link again if anyone has it. Here's cousin's info:
