I just did a survey for them and it said 25 dollars for the initial survey then 50 dollars more for the last survey, total of $75. :)

I signed up a couple months ago for a test they said they would be sending the product , I dont know if its the same, unfortunately I was expecting the product or an email and I did not get the addy. OP-- did you send pictures to the email addy that had the link for the dogcollar survey? I scanned and sent them but have not heard if they were acceptable..
How many have you sent?

Unfortunately my camera died that day so I sent in none.
