Many years ago this happened to me. I was going to redeem some points (I cannot remember the site) and a gal from Alaska's name and addy came up. I just deleted all of her stuff and then sent her an email telling her what happened. She had redeemed some of her points and her name and addy just stayed on there. She was very nice, emailed me back thanking me for getting rid of her name on there. I also told her to email the site and tell them.
