INFORMATION

You are invited to participate in a research study. This study involves your completion of a set of six questionnaires, including a newly developed questionnaire aimed at evaluating the relationship between various mood states (e.g., anxiety and depression) and sexuality (e.g., sexual interest and response). The other questionnaires include questions about your age, religion, marital status, alcohol and drug use, medical status, and use of medications, as well as questions about your sexual experiences (including number of partners, condom use, etc.), sexual functioning, sexual preferences, general tendencies to get depressed and to feel anxiety, and how you are likely to react in a variety of sexual and non-sexual situations. The questionnaires will take a total of approximately 40 minutes to complete.

COMPENSATION

We will send you a $10 check for your participation. For this purpose, we will provide you with an online form at the end of the questionnaires which you can use to email us your name and mailing address. We will not keep a record of your name and address, and we will have no way of connecting your email to the questionnaire. If you prefer to not give us your name and address, you can also choose to participate without being paid.

RISKS

We believe there is little risk in participating in this study, but you will be answering questions about your sexual feelings and behavior, your mood, and use of alcohol and drugs. Your answers will be treated as strictly confidential, and will be accessible only to those in the research group developing this questionnaire. There is a small possibility that having answered the questions, you may confront yourself with an issue that troubles you. If you have any questions that arise about your responses to the questionnaires, you are welcome to contact the principal investigator, Erick Janssen, Ph.D. (ejanssen@indiana.edu, tel: 812-855-7686) to discuss them in confidence. If you make such a contact, you will not need to reveal your identity unless you wish to do so.

FUTURE STUDIES

You will be asked on a separate, online form whether you might be willing to participate in further research. If you say yes, then you will be asked to identify yourself so that we can contact you.

CONFIDENTIALITY

For the duration of your visit to our website, you will be assigned a unique identifier. No identifying information about you or your computer will be recorded by the data processing software. If you email us your name and mailing address to receive payment, we will have no way of connecting your email to your questionnaire, and we will erase your name and address information from our files as soon as we have mailed you the $10 check. If you do not identify yourself by offering to be contacted about possible further research, then your answers will remain anonymous. If you do identify yourself, you will be asked to give your unique five-digit number and email address. This information will be treated as highly confidential. For a limited period of time, we will keep your contact information and a list that contains both your name and your questionnaire number. This information will be stored securely in the investigator’s office and will be made available only to persons conducting this study. All identifying information, including the list and your contact information, will be destroyed at the end of the project, in December 2005. Any e-mail messages you might send us will also be deleted after completion of the study. When the results from this study are reported, there will be no way of identifying you. In case you will contact us, for example with questions about the study or concerns about your participation, we wish to remind you of the lack of confidentiality of e-mail. In the Contact Section, you can find both e-mail addresses and phone numbers of the investigators.

PARTICIPATION

Your participation in this study is voluntary. If you decide to participate you are free to withdraw at any stage before completing the questionnaires. You are also free to refuse to answer questions.
