sears 10%/15%/20% rebate form needed - Big Big Forums

Hi everyone,

I am in desperate need of a copy of the rebate form from sears when they had their 10% rebate on one appliance, 15% rebate on two appliances, or 20% rebate on three appliances from around October 30th of 2005. It was program number 50755.
I sent in all my documents, but of course they never received it and they will not accept a resubmission without that form. I have checked online at sears.com/rebates but of course it is not listed and tried calling their phone number and no one can help. If you did this rebate and still have it lying around could you please let me know and get it to me by either mail or scan it in and send it to me. I do not care if you have your information written on it yet as I can delete it with my info. Please, please help me out as I bought 3 appliances and desperately need my 20% back. I will give anyone a reward if you want for this form. Thank you so much for any help.

I am sorry I can not help you, but when this happens to me I just fax or resend them my information that I kept copies of. Did you keep a copy of everything? Try calling and talking to someone else sometimes the first person you talk to has no clue. Sorry I couldn't help you with the form.

I have copies of everything but the form, which I have talked to a few people and they all tell me the same thing. They tell me that I need the form in order for them to process it. I have one portion of the form that was told to keep, but do not have a copy of the part where I enter my address information. I think they are giving me the run around because they don't want to give me the 20% rebate. So, if anyone made a copy for their own records and would be willing to get me a copy so I can write in my information I would be forever grateful.
Also, this is the first time I happened not to make a copy of my rebate form as I thought having the bottom section would be good enough proof, but of course it comes back and bites me in the butt because they do not receive it.
