Sweepstakes prizes: FOUR (4) GRAND PRIZES: A trip for two (2) to Oahu, Hawaii to participate in the American Heart Association walk on August 4, 2007 at Kapiolani Park and a Fitness Prize Package consisting of (i) $500 American Express gift check that could be used towards the purchase of fitness/walking attire, (ii) one hundred eighty (180) $4.90 Healthy Choice coupons, (iii) a one-year Platinum Local Area membership for Bally Total Fitness. Trip package includes roundtrip coach-class airfare for two (2) from a major airport near winner’s home (determined by Sponsor in its sole discretion) to Honolulu, HI; 5 days and 4 nights accommodations at a hotel determined by Sponsor in its sole discretion (single room, double occupancy); a full-size rental car for length of stay; and $1,000 spending money. If winner does not meet requirements for rental car, that portion of the prize will be forfeited in its entirety. If there is not a Bally’s within a 25 mile radius of the winner’s home, that portion of the prize will be forfeited in its entirety. Sponsor not responsible if the American Heart Association Walk is delayed, postponed or cancelled for any reason, in which event that portion of prize is forfeited in its entirety and no substitution will be provided except as in Sponsor’s sole discretion. Approximate Retail Value ("ARV"): $11,582. Actual value may vary based on airfare fluctuations and distance between departure and destination.
