If you're using Internet Explorer and having trouble getting the 'SUBMIT' button to work, try using a different browser...when I switched to Safari, the 'SUBMIT' button finally worked for me (which was much progress IMO)...however, I did still get the server error...but a couple refreshes later, I got the form to accept my request (as the post above shows)...GOOD LUCK and THX OP !!!

Yup, me too. Submit wouldn't work in IE8 nor Maxthon so downloaded/installed Safari. By the time I did that - all gone apparently.
