I know this is an older post, but I have something to add...

Things go much better for you if you go to these sites and print out the form. Then make an appointment with your doctor, and during the appointment, have them fill out their part, usually it isn't too long a form that they can do it quickly. Then when you get home, copy your w2's, or whatever form of income you have, and THEN send it yourself. I got answers within a couple of weeks from all mine.

Not that he is a bad doctor, as some people travel almost 2 hours to see him but, now he is providing a valuable service to the patients and their family and friends with the care and Medication they need when they are not in a position to afford such services.

I want to say thank you for these posts. I was diagnosed with Rheumatoid Arthritis three years ago and I am now completely disabled. I take low doses of chemo every week and have Remicade treatments every 6 weeks. The Remicade treatments alone cost around $2,500 each time. In the state of Wyoming, you can only receive Medicaid through SS disability if there is no other income in the house. There is absolutely no way that I could ever afford all of the meds that I take, but we can't afford my husband not to work so that I can get my meds. Hopefully I can find some help for my childrens health care as well, I have two kids under the age of 6.

I did this one because Hubby lost his job due to mass layoffs when Kellogg's closed their plant. There are tons of programs out there for displaced workers.... even if you aren't a displaced worker, there are programs where you can get money you can use.

For instance... I am eligible for a program that will pay for my Medicare part B. That's wonderful, because I NEED that coverage and cannot afford it. I am also eligible for the Medicare part "D" program, where they I will get prescription coverage for a percentage of the cost of the medication, and they will pay the first $600.00 of them.

thanks I needed that dental info!
I pay my own medicaid each month ($359 spend down program) because it pays for my meds that cost over $600 a month. But I cannot find a dentist in my area that accepts medicate. Now I will start looking where you posted

Sight for Students is a VSP charity that provides free vision exams and glasses to low-income, uninsured children. The program operates nationally through a network of community partners who identify children in need and VSP network doctors who provide the eyecare services.

More than 50,000 children each year receive a free comprehensive exam and corrective lenses if needed through Sight for Students. These are children who would otherwise not receive the eyecare and eyewear they need to do their best work in school and perform their best at play.

VSP founded Sight for Students in 1997 and was one of the original commitment-makers to America's Promise, which strives to bring together organizations dedicated to helping our nation's youth.
