http://www.rxassist.org/default.cfm is the site I use at work. It's the easiest way to find the correct form for the specific medication you are seeking assistance with. It allows you to search by medication, and gives you a link to the form you need. To make it a little easier on your physician and office staff, please complete as much of the form as you can, and sign the form before you bring it to the doctor's office. By doing this, we can just fill in our part, doc signs form and Rx and we can mail or fax it in directly. We don't have to call you for your income info, and signature. You'll get your medication much faster.

HELLO, TO ALL!!!!! OK, Y'ALL I NEED THE NECODERM PATCH SO THAT I CAN STOP SMOKING!!!! I NOW SMOKE YOU TO 2 PACKS A DAY. I HAVE TRIED THE OTHER NO-NAME PATCHES, THEY HAVE NOT HELPED ME TO STOP SMOKING. IT'S ETHER THE PACH BURNS MY SKIN OR IT DOES NOT STOP MY CRAVING AT ALL!!! IS THERE A PROGRAM THAT CAN HELP ME WITH THIS PROBLEM??????? I HAVE ASTHMA, BRONCITISS, AND CHRONIC LYRONGITUS (SPELLED WRONG). I REALLY NEED TO QUITE SMOKING. SOMEBODY HELP ME PLEASE!!!! THANKS FOR ALL YOUR HELP.!!!!!

I am bumping this up BIG TIME! My mom is on Fosamax for her osteoperosis. My mom is on this program & receives this medication in the mail. It does take a while to come so her orthopedic surgeon gives her samples to keep on hand. This drug is very expensive if you have to pay for it at the pharmacy.
One thing I do want to add is that a person doesn't necessarily have to be 'down & out' to get on this program. My mom is on Social Security & receives a company pension (she is also on Medicare & has a secondary insurance). My mom isn't wealthy but she isn't living in poverty, either.
Don't be discouraged if you think you may not qualify for this program. You won't know unless you try!

Since most of the smoking cessation patches are over the counter now, you might have a harder time getting them through a patient assistance program. The hospital we are affiliated with(and it's a small one) has a totally free smoking cessasion program through the pulmonary rehab. department. The program is totally free, they offer one on one, and support group meetings, the group meetings are not mandatory, but the one on one meetings are, so they can follow your progress. They supply the patches free of charge, and they don't care what your income is, they just want you to quit smoking. They only require a note from your doctor letting them know that it's safe for you to wear the patch. You might give your local hospital's pulmonary/respiratory therapy dept. a call to find out if they offer such a program. I think most hospitals do.
Another option, if the above isn't available to you, would be a drug called Wellbutrin (Bupropion), this drug was marketed just a few years back under the brand name Zyban, specifically for smoking cessation(interestingly, this medication has a side effect of *about* a 10 lb. weight loss!). It is prescribed more often now for depression, but is still used for smoking cessation. Anyway, you usually start the medication at least 2 weeks prior to your set quit date, to allow the medication to get into your system. Then continue on it for as long as you feel you need it. This is available through patient assistance programs, and is also sampled by reps. We always have a ton of it in the office. So if you don't have access to the patches through your hospital, you could try this route.
